Access your medical records

Medical records hold information about you. They are also sometimes called health records.

You'll have separate records for any NHS service you go to including your GP surgery, hospital, dentist or opticians.

How to get your record depends on which record it is.

You will need to complete an application for access to medical records,  SAR.

This can be collected at our Sandy Row Reception.

Please note that due to the high volume of requests for medical notes the avarage turn around time can be between 4-6 weeks from the date of request.
